Внутримозговое кровоизлияние при эклампсии

Аннотация
Классическая триада симптомов остается основой постановки диагноза «эклампсия». Тем не менее, ни одно осложнение беременности не бывает таким различным по клиническим проявлениям, неопределенности и сомнительности прогноза для матери и плода и по непосредственным причинам летальных исходов. Мы представляем описание клинического случая материнской смертности. У 17-летней первобеременной пациентки с анамнезом артериальной гипертензии при сроке гестации 31±32 недели развилась серия судорожных припадков и произошла утрата сознания. Диагностирована эклампсия, осложнившаяся острым нарушением мозгового кровообращения. Беременная была родоразрешена экстренной операцией кесарева сечения. В послеоперационном периоде родильница переведена в нейрохирургическое отделение для обследования и терапии. По данным компьютерной томографии было выявлено внутримозговое кровоизлияние с прорывом крови в желудочки мозга, окклюзионная гидроцефалия с уровня 4-го желудочка. Выполнено вентрикулодренирование по Арендту. Прогноз для жизни неблагоприятный. На 5-е сутки послеродового периода констатирована смерть больной. Мы считаем, что публикация и обсуждение подобных случаев должны способствовать улучшению понимания развития эклампсии и ее жизнеопасных осложнений. К счастью, эклампсия редка, процент частоты ее осложнений еще ниже. Но каждый подобный случай заслуживает детального и вдумчивого обсуждения. Ключевые слова: эклампсия, внутримозговое кровоизлияние.
